After someone dies, the executor or personal representative (the person in charge of wrapping up the deceased person’s affairs) needs to notify creditors of the death and close the deceased person’s credit accounts.
Use this form to inform credit card issuers, department stores, banks, mortgage companies and other businesses with which the deceased had an account.
Important to Know
If a formal probate court proceeding is conducted, you’ll also have to follow court rules for notifying creditors. Check with the lawyer handling the proceedings or ask the probate clerk at the county courthouse for the rules.
