Notice of Revocation of Power of Attorney

  • You want to revoke your power of attorney before the termination date set out in the document.
  • Your power of attorney has ended as specified in the document, but you want to be absolutely sure that all institutions and people who received it know that it is no longer in force.

  • Revoke a power of attorney with one simple form

    If you've created a power of attorney, it's sometimes necessary to revoke it. When you do so, it's important that all parties are notified and your state is aware that the power of attorney is no longer in effect. You can use this form in two situations:

    • You want to revoke your power of attorney before the termination date set out in the document.
    • Your power of attorney has ended as specified in the document, but you want to be absolutely sure that all institutions and people who received it know that it is no longer in force.

    Important to Know

    • This form must be notarized and (in some states) witnessed to be legally valid.
    • If you recorded your Power of Attorney with the local land records office, also record the Notice of Revocation.
