Simple forms to use that cover bases. As a word of advise make sure you follow the date parameters and delivery method as instructed. You don't want to make that mistake. -

Why is this called a “kit”? All I received were forms with erasable indications as to what went in each blank. These indications caused trouble when typing in my answers. Also, the 60-day notice did not provide flexibility regarding the move-out date and other terms the landlord might wish to offer. -
60-Day Notice / Proof of Service

I ended up hiring 2 different process servers on Yelp to deliver the notice in order to obtain the Proof of Service. The notice was delivered by one of the servers. I have yet to see what happens after the 60 Days on 2/17/2021. I will need to follow-up with a forwarding address for the security deposit after inspection of the property. That will be pending.
