Will your child be going out of the country with just one parent, or with another relative or friend? If so, you’ll need proof that both parents have given their consent to this travel. A child who doesn’t have written consent is unlikely to be permitted to travel outside the United States with someone who is not a parent or legal guardian.

You can provide that written proof quickly and easily by completing this form, which makes it clear that you, the parents, have authorized another adult to travel with your child to another country. You’ll then sign the international travel authorization in front of a notary public to show that your signature is genuine. Airline staff and border officials, presented with this signed and notarized document, should have no problem with allowing your child to travel outside of the United States.

Important to Know

  • You should sign your foreign travel authorization in front of a notary public.
