    • Once a contract has been signed, it can be changed only if all parties agree and sign an amendment. Use this form for amendments to most types of contracts.

    Important to Know:

    • Don’t use this form to amend a lease. Instead use "Amendment to Commercial Lease."
    • Don’t use this form to amend a real estate purchase contract.
    • If agreed-upon changes to a contract are numerous or complex, create a new contract.
